XHTML live Chat by alexander kohlhofer is a simple Shoutout box for websites. XHTMLChat required PHP and mySQL. Adjust db.php with you mySQL credentials. Then run initBD.php. After this you should be able to chat away at index.html, follow the step by step tutorial:

Download AJAX / XHTML Script, unzip the file.
Execute db.php files and enter your MySQL Server Address, MySQL Database, MySQL User, and MySQL Password
Save the changes, upload the entire folder to your web directory.
To install the script, run initBD.php ( Example : http://www.yourdomainname.com/initdb.php )
